It only appears in multiplayer runs. It is Sly, so if it is discarded from your hand before the end of your turn, it is played for free. Once it is in play, every Attack card another player plays gives you 1 Block (2 upgraded); the code checks the card's type, not what it hit. The Block is unmodified by Dexterity.
When to take Sneaky
Take it in a co-op run where your partners play a lot of Attacks, and discard it into play rather than paying 2 Energy. A Shiv-heavy partner can trigger it ten or more times a turn.
