Any card counts: curses, event cards and cards you did not choose all advance it.
Stops appearing: it is not offered from the act 3 treasure chest onward, floor 41 solo, 38 in multiplayer, but one already owned keeps working.
Unlock: it unlocks at Ancients.

When to take Book of Five Rings
Best in act 1 for a deck that takes a card from nearly every reward, worth two or three heals over the act. Loses value in a lean deck that skips rewards, but beats Blood Vial.
