The bonus is added to the hit before Strength and Vulnerable are worked in, so a base 4-damage Shiv becomes 8 or 10 flat. Copies stack. Shivs thrown at every enemy under Fan of Knives get the bonus on each target.

When to take Accuracy

A blank card until you own two or more Shiv generators like Blade Dance or Infinite Blades. Then it is one of the best turn 1 plays, and Shiv count multiplies its upgrade bonus.
