The upgrades are shown on screen and last only for that fight; your deck outside combat is unchanged. Already upgraded cards are skipped, so it never wastes a pick on them.

When to take Stone Cracker
Best in Act 1 or early Act 2, while most of your deck is unupgraded, since 2 of 15 cards is a real hit rate on a small deck. On a bloated 35-card deck it is background noise.
