An Act 1 boss normally drops Gold, maybe a potion and a card reward, with no relic. With this relic, the reward screen for that boss gains two relic drops, pulled from the front of your relic pool like elite rewards. It fires once, only while you are in the first act, then the relic greys out.

When to take Lava Rock
Always take it when offered in Act 1, since two extra relics before Act 2 is among the strongest things an Ancient can give. It has no value once the Act 1 boss is already behind you.
